Class M antibodies appear first — this can be observed several days after the onset of symptoms. Almost simultaneously or shortly after them, IgG appears (in more than half of patients already in the period of 8-14 days from the onset of clinical signs of infection, and in the period of 15 days or more — in 99% of patients). In terms of the level and dynamics of the concentration of antibodies in the blood, the humoral immune response varies individually, including depending on the severity of the disease. The level of IgM decreases to undetectable, usually within one, less often two months from the onset of the disease, while IgG antibodies can remain in the blood for a long time (3-5 months or more), performing a protective role.

The four structural proteins that encode the RNA of the SARS-CoV-2 virus are spike (S), envelope (E), nucleocapsid (N), and membrane (M). Structures that protrude from the surface of the virus like spikes and give a resemblance to the crown are formed by the spike (S) protein. The S protein consists of two subunits, one of which contains the receptor-binding domain (RBD), through which the virus binds to receptors on human respiratory epithelial cells upon infection. Nucleocapsid and spike proteins cause the most pronounced antibody response during infection. The detection of specific antibodies to these proteins is used as immune evidence of the body’s past contact with the new coronavirus. At the same time, the neutralizing activity of antibodies produced against SARS-CoV-2 is mainly correlated with antibodies to the S-protein (which is therefore the main target in vaccinology).

Vaccine preparations using the coronavirus S-protein as a target (including the vector Gam-COVID-Vac — Sputnik V, Sputnik Light), or vaccines based on S-protein mRNA, as well as inactivated whole-virion vaccines (including CoviVac ) also induce the production of antibodies to the SARS-CoV-2 S protein.

What is the purpose of quantitative determination of IgG antibodies to SARS-CoV-2 S-protein

The detection of specific antibodies to the SARS-CoV-2 class G virus indicates recent or past exposure to the virus. Therefore, such tests are used in complex diagnostics in the presence of clinical suspicions of infection with a new coronavirus or its complications. Primary method for laboratory confirmation of acute COVID-19 infection- detection by polymerase chain reaction (PCR) of the presence of virus RNA in a biomaterial taken from the respiratory tract, usually a swab from the nasopharynx and oropharynx. However, the information content of RNA testing depends on the sufficiency of the virus content in the material of the selected localization at a particular stage of infection, as well as the quality of the material taken. The most informative PCR studies of smears from the oropharynx and nasopharynx in the first 1-5 days from the onset of clinical manifestations of infection. At later dates (more than 1-2 weeks), it is advisable, in addition to PCR testing of smears, to use a blood test for the presence of specific antibodies produced by the body in response to SARS-CoV-2 infection.

Assessment of the level of IgG antibodies to SARS-CoV-2 can also be used to identify immunological evidence of past (including subclinical or asymptomatic infection) to assess the probable immune status of the examined person in relation to this virus, contact tracing, population epidemiological studies.

Since the majority of registered and developed SARS-CoV-2 vaccines target the production of antibodies to the S-protein, the quantitative assessment of the level of these antibodies can be an informative method for assessing the dynamics of the immune response to vaccination with such drugs. In particular, this test can be used to evaluate the response to the use of a vector vaccine with the inclusion of the S-protein Gam-COVID-VAK — Sputnik V, Sputnik Light, as well as whole-virion inactivated vaccines (including CoviVac produced by the Federal Research Center named after M. P. Chumakov) or vaccines based on S-protein mRNA (in particular, Pfizer-BioNTech and Moderna vaccines or others). The test is not applicable for assessing immunity after vaccination with EpiVacCorona.

Understanding which tests are better to take and why

In a semi-quantitative test, the laboratory does not count antibodies in absolute units, but reads the optical density of the blood serum in which the antibodies float. As a result of such an analysis, there will be a number — the positivity coefficient (CP), which means how many times the optical density of the resulting solution is higher than the optical density of the serum, in which there are no antibodies. The values ​​here are usually small, it can be 4 KP or, for example, 16 KP, but anything above one indicates the presence of antibodies.

In the quantitative method, the laboratory calculates how many units of antibodies are contained in a microliter of blood serum, the values ​​are given in BAU/ml. It is this analysis that is now most popular with clients of private clinics. The values ​​in this analysis are usually higher, the numbers vary from 10 to several hundred.

Experts have not yet come to a consensus on what titer is sufficient to protect against coronavirus infection, but the first conclusions are already there.

— Currently, according to current guidelines, the presence of antibodies is not a contraindication to vaccination against a new coronavirus infection. There is no reliable data on the «protective» or «sufficient» level of antibodies. In addition, the validation of test systems has not taken place in the world — there is no single reference value that can be used by all diagnostic laboratories. Thus, today it is recommended to be vaccinated at any antibody titer and undergo revaccination six months after the first immunization or six months after recovery, says doctor Dmitry Prokopyev.

Epidemiologist Alexander Vygonyailov agrees with Dr. According to him, now there are isolated studies that suggest that a titer of 300–350 BAU / ml may be sufficient for protection, but it does not provide 100% protection and can quickly decrease, so the vaccine must be repeated every six months.

— Antibodies will still decrease, just in this case you don’t need to use a double vaccine, it’s enough to put Sputnik Light, — the doctor explained. — There will be no overdose of antibodies, it cannot be a priori. This cannot cause some kind of autoimmune process, because the antibodies are specific, directed against a specific coronavirus — the more there are, the more intensively they will rush to destroy it.

For those who have the opposite situation — less than six months have passed after vaccination, and the antibody titer is already low — the epidemiologist advises to still meet the standard period.

“Vaccination too often can lead to exhaustion of the immune system, it will simply stop responding,” the specialist added. — Therefore, it is not necessary to be vaccinated earlier than six months later, until the situation does not allow later than six months.
